Does anyone know the source of the quote that goes something like:

i.e., the fault is in the system...

It's been going round my head these days. Really bothering me. Any help appreciated.

Also, a maxim I always liked (probably because it justifies sloth):

The more efficient the system, the more sensitive it is to shortage.
