MAD is mutually assured destruction. Missile were fired to take out hardened silos and their hit rate was 60 percent sith some failures. now hit rate is 100%
So we built ENOUGH so that even if we were attacked first we could still take our every city.
now, with GPS there wont be any left.
on top of that the incoming missile is now sensitive to oncoming object and will detonate (in case the missile is supposed to be taken out gets launched a minute earlier, it is still taken out.

so, instead of MAD

grabby, I'm not quite sure how that relates to folks being upside down on an auto loan, but I'm sure you could explain it.
As to the scenario... I suppose anything's possible, but I'm much more worried about a big meteor ending everything than I am about the likelyhood of a superpower full nuclear launch. (not likely!) I don't disagree about the probability concerning a single city getting hit with a nuke, but I'd expect it to be delivered by unconventional means, and not the product of a superpower; nor would I be willing to make the assumption that the city in question would be US or Israeli. Its just too hard to do with a missle, and too easy to do in other ways.
