Are we foolishly optimistic to say if Nuclear Terrorism has not happened in the 26 years since the USSR fell apart that the fear was totally overblown and never a real threat?

Nation states are not going to use them against anyone that can retaliate. There are groups of people who would not hesitate to use one, like ISIS if they had one.
